New Patients

Welcome New Patients! We look forward to seeing you in our office and meeting your family. Please visit our Meet Our Team page for information about our physicians.

How our office works: For your convenience, we begin answering our phones on Monday-Friday at 7:30am and on Saturday at 8:30am. Our staff and phones remain available to our patients during the lunch hour. We offer both morning and afternoon same day appointments for sick patients and Saturday appointments for sick patients only and recommend you call early for the best availability. Our after school hours are limited. Well visit appointments are typically scheduled in advance. Please be prepared to provide your driver's license, insurance card, and copay during Check In at each appointment. Please review our General Information handout for additional helpful information regarding our practice.
Check In: As you enter the office, please check the posted blue Check In signs located to the right of the first window. Arrows will direct you to the appropriate window for the physician you are seeing that day. We use separate Check In windows for each physician in the office and the signs are updated daily depending on the office schedule. In addition, signs with the appropriate physician's name are also posted over each window.

Check Out: Your Check Out procedure will occur at the same window you used for Check In. We find that most children anticipate selecting the sticker of their choice at the end of their visit. Our current sticker selection is posted daily at each window.

Waiting Room Areas: We provide separate well and sick waiting room areas. These sections change depending on the season, so be sure to check our posted signs to determine the appropriate waiting area for your appointment.

New Patient Paperwork: For your convenience, we have provided links to the forms that need to be completed prior to each new patient visit in our office. Please review, print, and complete the forms at your leisure prior to your appointment day. This will allow you the opportunity to focus your attention on the paperwork and will speed up your check-in process. Don't forget to bring the completed forms to your appointment.

We require completion of the following forms:
