Our flu clinic page will be updated each season with appropriate dates and forms.
- Flu questionnaire forms will be available near the elevators close to the suite entrance.
- Check In will occur at the table just prior to suite entry.
- Physician Ordering: You will be directed to the front desk window with the appropriate physician who will order your child's flu vaccine, then direct you to the waiting area.
- Clinical staff will take your child to an exam room as soon as an exam room is available.
Prior to your appointment, please print out and complete the appropriate forms listed below.
(Forms for the current flu season will be available as soon as the CDC posts their current flu vaccine recommendations.) Be sure to bring the completed forms with you to your child's flu vaccine clinic appointment.
