Welcome New Patients!

As you enter the office, please look for the blue Check In sign located above the first window. Signs are updated daily to list which physicians are scheduled to work that day. 

Your Check Out procedure will occur at the far right window as you face the receptionist windows. We find that most children anticipate selecting the sticker of their choice at the end of their visit. Our current sticker selection is posted daily at the Check Out window.

We provide separate well and sick waiting room areas. These sections change depending on the season, so be sure to check our posted signs during each visit to determine the appropriate waiting area for your appointment.

For your convenience, we have provided links to the forms that need to be completed prior to each new patient visit in our office. Please review, print, and complete the forms at your leisure prior to your appointment day in addition to the Phreesia documents you were texted or e-mailed. This will allow you the opportunity to focus your attention on the paperwork and will speed up your check-in process. Don't forget to bring the completed forms to your appointment.
